Textbook Refund Form
This form is to be filled out if textbook and or other materials not listed on the book list was required to be purchased for a class. Please be sure to include all receipts and syllabus for each class indicating items purchased and requirement. These materials are provided as part of the student athlete scholarship and are to be returned to the Athletics Department during book buyback/finals week.Student Promotional Activities Form
This form is to be filled out if a student athlete(s) are participating in any type community service or this form can also be filled out by outside parties wanting to request Student athlete appearances at community service events. This form must be approved by the Compliance office prior to any and all community service/volunteer work. For questions, please contact thorner@huntsman.utah.edu.
